category description
Up-To-Date Kindergarten students who have completed / are up-to-date on required immunizations, defined as 3 doses of polio vaccine; and 4 doses of DTP, or combination of DTP and diptheria-tetanus toxoids-containing vaccine; and 1 or more doses of measles, mumps and rubella-containing vaccine, separately or combined, on or after 1st birthday and 3 or more doses of hepatitis B vaccine; and 1 or more doses of varicella vaccine or physician-documented varicella (chickenpox) disease
Conditional Kindergarten students who do not meet the immunization requirements because (1) they are "in process" and have not received all required doses, (2) they have a temporary medical exemption or (3) they are a transfer student and school has not yet received their immunization documentation
Permanent Medical Exemption (PME) Kindergarten students with a Permanent Medical Exemption (PME) granted upon the filing with the school of a written statement from a licensed physician to the effect that the physical condition of the student or medical circumstances relating to the student are such that immunization is permanently not indicated
Overdue Overdue and subject to exclusion from school for non-compliance (new category starting in 2015-2016)
DTP Kindergarten students who have received 4 or more doses of any diphtheria and tetanus toxoids and pertussis vaccines including diphtheria and tetanus toxoids, and any acellullar pertussis vaccine (DTaP/DTP/DT)
POLIO Kindergarten students who have received 3 or more doses of polio vaccine; oral polio vaccine (OPV) or inactivated polio vaccine (IPV) or any combination of these
MMR Kindergarten students who have received 2 doses of measles-mumps-rubella vaccine both at or after student's first birthday
HEPB Kindergarten students who have received 3 or more doses of Hepatitis B vaccine
VARI 2 or more of Varicella.  In contrast, from 2000-2001 through 2018-2019, the requirement was for 1 or more doses of Varicella vaccine or a history documented by a physician of having had chickenpox disease. History of disease is currently requested to be reported as a permanent medical exemption.
